The Plague Pistol is an Epic-only sidearm that stands apart from every other pistol in Windrose thanks to its extraordinary 40% lifesteal on every shot. Dealing 390 Pierce damage, each round fired actively heals the wielder for a significant portion of the damage dealt, turning offensive pressure into a survival tool. This makes the Plague Pistol uniquely valuable in prolonged fights where attrition would normally force a retreat. No other ranged weapon in the game offers this level of self-sustain, and it fundamentally changes how you approach combat encounters.

Building around the Plague Pistol means maximizing every point of damage to amplify the healing return. The Marksman talent branch is essential here, as critical hit bonuses and damage multiplier nodes directly increase the amount of health recovered per shot. Equipping the Marksman's Rig armor set adds 15% ranged damage, which also feeds into the lifesteal calculation, effectively boosting your healing output by the same percentage. The Take Aim! buff from a Cutlass melee strike pushes this even further, creating windows where a single Plague Pistol shot can restore a substantial chunk of your health bar.

Because the Plague Pistol only exists in Epic rarity, acquiring one requires dedicated farming of high-tier loot sources, elite pirate encounters, or rare treasure chests. Once obtained, it becomes a cornerstone of any survivalist loadout. Use Iron Bullets or Bullet Arborum to maximize damage and, by extension, healing per shot. In group content, the Plague Pistol allows you to play aggressively as a frontline ranged fighter, absorbing chip damage that would send other players scrambling for healing items.
